This notice describes how medical information about you may be used, to whom it may be disclosed and how you can get access to this information. Please review it carefully.

The hospital cares about your privacy as much as we care for your health. This privacy notice is subject to the implementation of the data privacy and data protection laws which is Republic Act No. 10173 or the Data Privacy Act of 2012 (DPA), its implementing Rules and Regulations, and other relevant policies, including issuances of the National Privacy Commission. This will explain how our hospital uses the personal data we collect from you, our clients and patients when you use our website.


Personal information – refers to any information whether recorded in a material form or not, from which the identity of an individual is apparent or can be reasonably and directly ascertained by the entity holding the information, or when put together with other information would directly and certainly identify an individual.

Sensitive personal information – refers to personal information:

1.) About an individual’s race, ethnic, origin, marital status, age, color, and religious, philosophical or political affiliations;

2.) About an individual’s health, education, genetic or sexual life of a person, or to any proceeding for any offense committed or alleged to have been committed by such person, the disposal of such proceedings, or the sentence of any court in such proceedings;

3.) Issued by government agencies peculiar to an individual which includes but not limited to, social security numbers, previous or current health records, licenses or its denials, suspension or revocation, and tax returns; and

4.) Specifically established by executive order or an act of congress to be kept classified.

The hospital collects the personal information you provide us which may include:

      ✓Other information about you to help us get to know what health services we can provide you, such as your chief complaint, past medical history

We collect your personal information through many ways such as interviews, from application forms, or through e-mail and websites.

Hospital Admission

      a.) Upon admission in the hospital, personal information collected in our inpatient form such as name, permanent address, present address, age, sex, civil status, religion, contact number, birth date, birthplace, nationality, father’ and mother’s details, spouse details and personal information of person / parties responsible for your account;

      b.) During the duration of patient stay – medical information collected by our medical staff such as medical history, medical diagnosis, laboratory results, vital signs, medicine intake, and medical procedures done.


      a.) Upon submission of research papers, personal information collected in our registration and application form includes the principal investigator and team member(s) names, business, address and contact information such as e-mail address, mobile and telephone number.

Human Resource Development

      a.) Before employment to the institution – personal information collected in the personal information sheet (201 file) include applicant(s) name, age, sex, city and provincial address, date and place of birth, civil status, religion, height, weight, dialects and language spoken, hobbies, contact information (telephone, mobile phone number and e-mail address), family members background details (name of the parents and sibling(s), name of the mother and father in law, age date of birth, their occupation), educational attainment (elementary to college, post graduate course, seminars and studies attended, government identification numbers (SSS, Pag-Ibig, Philhealth, PRC license number), working experience(s) and character references;

      b.) During the duration of employee’s stay as personnel of the institution – personal information collected includes change of status (if subsequently married), list of dependents and any change in contact information address.


     a.) Upon entry and exit to the institution – requires to deposit a proof of identity (ID) for verification purposes. For those vehicles, we take note of your vehicle plate number. Video footage of your vehicle is also being recorded via CCTV system installed in entry and exit points of the institution.


The main purpose for obtaining your personal information is for providing medical services.


      The institution uses the patient information in relation to the health services we provide and ensure that appropriate treatment will be made to you as our patient.


      The institution collects the information mainly to identify, supervise, coordinate and monitor the implementation of the institution’s research programs.

Human Resource

      a.) Before the employment to the hospital – the institution collects your personal information mainly to screen, document and verify applicant(s) eligibility for initial employment including the verification of qualifications and for background checking. We also collect the information in administering employee’s pay, statutory deductions, entitlements and benefits;

     b.) During the duration of your stay as personnel of the institution – the institution collects personal information primarily to update employee(s) 201 file and used in administering employee’s pay, statutory deductions, entitlements and benefits.


      The institution collects the information mainly as security measure, which can help us to investigate any untoward incident that may happen in the institution, and generate statistics useful for planning and service improvements purposes.


The following are to whom your personal data will be shared:

     a.) Healthcare professionals such as doctors, nurses;

     b.) All department or sections in the hospital who will need to your personal information to perform its function; and

     c.) All the employees, staff or personnel who need to access your personal information to perform their duties.


Only those authorized personnel have access to your personal information in our files. We take appropriate organizational, physical and technical security measures to protect your data against unauthorized access to or unauthorized alteration, disclosure or destruction of data as well as against any other unlawful processing. All the personal data or information gathered will be stored in accordance with period guidelines and limitations provided by the Department of Health (DOH) for retention of medical records.
